In order to care for Europeans in British India, the British East India Company founded the Indian Medical Service (IMS) in the year 1764. The British Government announced a proposal on May 9, 1822, to replace the positions of native doctors in the Presidency of Bengal's civil and military establishments. On June 21, 1822, they established "The Native Medical Institution" with the aim of achieving the aforementioned goal and teaching medicine in the local tongue. A medical college was advocated in 1833 by the William Bentinck Committee "for the education of the natives". As a result, on January 28th, 1835, a new medical college known as the Medical College Kolkata was founded. Its goal was to educate local youths from all castes and religious backgrounds.

Only twenty pupils were chosen in a preliminary exam out of around one hundred applicants. After completing the course, students were required to take a final exam, and those who passed were given "President of the Committee of Education certificates," which allowed them to practice medicine and surgery. As "Native Doctors," they were qualified to apply for positions in the government. Madhusudan Gupta, a member of the inaugural batch of students, performed the first dissection of a human corpse in 1836, marking a significant turning point in the history of medical education in India. In West Bengal, today, January 10, is recognized as Medical Education Day. Umacharan Seth, Rajkrishna De, Dwarakanath Gupta, and Nabin Chandra Mitra were all well-known first-year students.

The first group of students who took the final exam on October 30, 1838, succeeded. They then qualified to perform surgery and medicine. Ram Kamal Sen and Dwarkanath Tagore, two prominent Calcutta residents at the time, encouraged medical education by offering scholarships and awards to students who excelled in their academic studies. Dwarakanath Tagore provided funding to send four outstanding college students to England. Gopal Chandra Seal, DwarakaNath Bose, and BholaNath Bose were among those who passed the MRCS (Member of the Royal College of Surgeons) exam in 1846 and then went back to India to join the Medical Service. One of them stayed behind, earned an MD from the University College of London, and became the first Indian to pass the test for the Indian Medical Service. In due course, the college produced numerous notable alumni, including Sir Nilratan Sarkar, U N Brambhachari, Bidhan Chandra Roy, Balai Chand Mukhopadhyay, Ram Baran Jadab, etc. Another significant day in the history of Medical College was June 29, 1883. On that day, Kadambini Ganguly, a Bengali Brahmo, was admitted as the institution's first female student. The government gave all female students in 1884 grants worth Rs 20 per month. These scholarships were given to Bidhu Mukhi Bose and Virginia Mary Mitter.

MotiLal Seal donated the land on which the college's structures were erected in May 1835. The oldest building of the Medical College is Medical College and Hospital Building(also known as the MCH Building). Later on different hospital buildings were also established in the college campus namely Eden Hospital (1881–82), Ezra Hospital (1887), Shayma CharanLaha Eye Hospital (1891), Prince of Wales Surgical Block in 1911, Sir John Anderson casualty block, David Hare block etc.
